The following terms and conditions apply to your participation in any patient support program (Program) sponsored by Ipsen Australia Pty Ltd (Ipsen) and managed by Partizan Worldwide Pty Ltd (Partizan) (‘us’, ‘we’ or ‘our’).
Please read these Terms and Conditions and the Ipsen Assist Privacy Policy. It is your right to refuse to agree to the terms and conditions outlined here; however, if you do not agree, you may not be able to participate in the Ipsen Assist Program. If you have any questions or require additional information, please speak to your doctor, or contact the Ipsen Assist Program Coordinator on 1800 477 366.
By enrolling into and using the Ipsen Assist Program, you agree to the following terms and conditions:
1.Use of the Program
You may only access and use the Program in accordance with these Terms and other instructions we provide you from time to time. You must not use the Program to upload or use any content that breaches any law or infringes a person’s intellectual property or other rights, or otherwise in a manner which is reasonably likely to cause loss or harm to any person or to jeopardise the health or safety of a person.
You understand and acknowledge that this Program does not provide medical treatment or advice and does not replace your usual medical care. You must contact your Healthcare Professional for any queries regarding your treatment or condition.
2.Eligibility
You confirm that you are eligible to participate in the Program, including that you are being prescribed an Ipsen product associated with the Ipsen Assist Program. If you are unsure, please contact Ipsen Assist for further information. We reserve the right to exclude participants who are not eligible to use the Program.

We reserve the right to alter or cancel any services under the Program, or the Program itself, at any time.
4.Ownership of the Program
The Programs and all their content, including any intellectual property rights, are the sole property of Ipsen. You are granted a non-exclusive licence to use and access the Programs for your own personal use, but not for any other purpose.
5.Privacy and consent
We will manage your personal information (including, where applicable, sensitive health information) in accordance with our Privacy Policy and our obligations under the Privacy Act 1988 (Cth). We take all reasonable steps to ensure your personal information is protected from misuse and loss and from unauthorised access, modification or disclosure. However, to the extent that the Program is delivered on the internet, which is inherently insecure, we cannot provide any assurance regarding the security of transmission of information you communicate to us online. Accordingly, any personal information or other information which you transmit to us online is transmitted at your own risk. If you have any questions, concerns or a complaint regarding the treatment of your privacy, please contact us using the information set out below.
You consent for your personal information (including sensitive health information) to be collected, used and disclosed as outlined in the Ipsen Assist Privacy Policy.
You give permission for the Ipsen Assist Program Coordinator or an Ipsen Assist nurse to contact you by phone, email, SMS, websites or via digital applications for the purposes of facilitating the program and evaluating the program for quality improvement purposes.
You give permission for the Ipsen Assist Program Coordinator or an Ipsen Assist nurse to contact your Healthcare Professional for the purposes of evaluating the program for quality improvement purposes.
If applicable, you give permission for an Ipsen Assist Program Coordinator to contact your nominated medical clinic, community pharmacy and/or hospital pharmacy to discuss the ongoing administration of your prescribed Ipsen medication with your doctor or practice nurse, AND/OR to train you/your nominated carer to administer your prescribed Ipsen medication.
You understand that you have the right to opt out of the Ipsen Assist program at any time by contacting the Ipsen Assist Program Coordinator or your specialist.
You give permission for the Ipsen Assist Program Coordinator, Ipsen , or a third party acting on its behalf, to contact your Healthcare Professional should you experience an adverse event.
6.Termination
You have the right to opt out of the Ipsen Assist Program at any time by contacting the Ipsen Assist Program Coordinator or writing to the Ipsen Assist Program at support@ipsenassist.com, or by contacting your Healthcare Professional.
We may also suspend or cancel your access to the Program at any time without advance notice and for any reason, including if you breach these Terms or for the maintenance of the Program, although we will use reasonable efforts to provide you with notice wherever practicable.
7.Conflict
As part of Ipsen Assist, Partizan is paid to provide the Program. If you have any concerns about potential conflicts of interest, you are encouraged to discuss these concerns with your treating Healthcare Professional.
8.Governing law
These Terms are governed in accordance with the laws of Victoria, Australia.
9.Changes to these Terms
We reserve our right to modify these Terms at any time, including Program eligibility requirements. We will use reasonable endeavours to ensure that you are alerted to changes, such as contacting you via email. Your subsequent continued participation in of the Program after such changes have been made will constitute acceptance of the new Terms.
We may also update, improve or otherwise modify the software and the Programs from time to time. If that happens, you may need to download or install updates of the software in order to continue using the Programs.

The Ipsen Assist program has been designed to comply with the Australian Privacy Principles (APPs) and is committed to managing personal information in an open, transparent and secure way.
Ipsen Pty Ltd (Ipsen) has engaged Partizan Worldwide Pty Ltd (Partizan Worldwide) to collect and hold your information in order to administer the patient support program services. If you choose not to provide the information requested, it may not be possible for the Ipsen Assist Coordinator to contact you, provide program services or finalise patient enrolment into the Ipsen Assist program.
In order for the Ipsen Assist program to provide participants with program services, some personal information, such as your name, age, address and telephone number, your nominated Healthcare Professional, as well as relevant medical information will need to be collected.
When collected, sensitive personal information will be used by the Ipsen Assist Coordinator to contact patients and Healthcare Professionals, and create an enrolment record electronically; and will be entered into the Ipsen Assist program registries, databases and web-based systems hosted in Australia.
Personal sensitive information provided may be disclosed by Partizan Worldwide to the patient’s nominated Healthcare Professional(s) or to third party individuals or organisations contracted or involved in the administration of the Ipsen Assist program located within Australia such as:
Partizan Worldwide will also disclose de-identified sensitive personal information to Ipsen, or a third party acting on its behalf, for complaint investigation, audit purposes, regulatory and adverse event reporting or as required by law.
Ipsen has a standard operating procedure by which all adverse events experienced by patients taking an Ipsen drug, when notified by an employee, contractor or any third party, must be collected, stored and processed, and reported to international regulatory authorities where required.
Ipsen, or a third party acting on its behalf, may contact a patient’s Healthcare Professional in order to collect further information related to an adverse event. Ipsen may share the de-identified sensitive personal information with its parent company the Ipsen Group, in France and the United Kingdom.
De-identified information relating to a patient’s participation in Ipsen Assist programs may also be collated into a report for Ipsen. Ipsen may use this information to:
Ipsen may also share de-identified information with its parent Company or its affiliates regarding how the Ipsen Assist programs are working on an aggregated basis. This information does not identify patients individually. Any personal information collected by Ipsen will be handled in accordance with Ipsen’s Privacy Policy.
Partizan Worldwide (or third party individuals or organisations contracted or involved in the administration of the Ipsen Assist program) will retain the sensitive personal information collected for the period of your participation in the Ipsen Assist program and after your participation in the program has ended for the purposes of reporting, audit and as required by law. Retention of sensitive personal information is also regulated by medical legislation. Any sensitive personal information relating to adverse event reporting must be retained for twenty years after the finalisation of product commercialisation, discontinuation of development, out licencing or other action whereby Ipsen have no further requirement for monitoring safety of the product.
At your request, Partizan Worldwide (or third party individuals or organisations contracted or involved in the administration of the Ipsen Assist program) will update your personal information as it changes during the course of participation in the Ipsen Assist program.
