Cohere Patient Support Terms of Use and Privacy Policy
The following Terms and Conditions and Privacy Policy apply to your participation in any patient support program (Program) delivered via the Cohere Patient Support mobile application that are managed by Partizan Worldwide Pty Ltd (Partizan) (‘us’, ‘we’ or ‘our’).
Please read these Terms and Conditions and the Privacy Policy that follows. It is your right to refuse to agree to the Terms and Conditions and/or the Privacy Policy outlined here; however, if you do not agree, you may not be able to participate in the Program.

Cohere Patient Support Terms and Conditions
By enrolling into and using the Program via the Cohere Patient Support mobile application, you agree to the following Terms and Conditions:
1. Use of the Program
You may only access and use the Program in accordance with these Terms and other instructions we provide you from time to time. You must not use the Program to upload or use any content that breaches any law or infringes a person’s intellectual property or other rights, or otherwise in a manner which is reasonably likely to cause loss or harm to any person or to jeopardise the health or safety of a person.
You understand and acknowledge that this Program does not provide medical treatment or advice and does not replace your usual medical care. You must contact your Healthcare Professional for any queries regarding your treatment or condition.
2. Eligibility
You confirm that you are eligible to participate in the Program, including that you are being prescribed a product associated with the Program. If you are unsure, please contact your Healthcare Professional for further information.
We reserve the right to exclude participants who are not eligible to use the Program.

We reserve the right to alter or cancel any services under the Program, or the Program itself, at any time.
4.Ownership of the Program
The Programs and all content, including any intellectual property rights, are the sole property of Partizan Worldwide. You are granted a non-exclusive licence to use and access the Programs for your own personal use, but not for any other purpose.
5. Privacy and consent/strong
We will manage your personal information (including, where applicable, sensitive health information) in accordance with our Privacy Policy (see below in this document) and our obligations under the Privacy Act 1988 outlines the Australian l Privacy Principles.
We take all reasonable steps to ensure your personal information is protected from misuse and loss and from unauthorised access, modification or disclosure. Refer to the privacy Policy later in this document for more details on how we protect your personal information. You consent for your personal information (including sensitive health information) to be collected, used and disclosed as outlined in the Privacy Policy.
You give permission for the Program Coordinator or a nurse to contact you by phone, email, SMS, or via this application for the purposes of facilitating the program and evaluating the program for quality improvement purposes.
You give permission for the Program Coordinator or a nurse to contact your Healthcare Professional for the purposes of evaluating the program for quality improvement purposes.
If applicable, you give permission for a Program Coordinator to contact your nominated medical clinic, community pharmacy and/or hospital pharmacy to discuss the ongoing administration of your prescribed medication with your Healthcare Professional or practice nurse, AND/OR to train you/your nominated carer to administer your prescribed medication.
You understand that you have the right to opt out of the program at any time by contacting the Program Coordinator or your Healthcare Professional.
You give permission for the Program Coordinator or a third party acting on its behalf; to contact your Healthcare Professional should you experience an adverse event.
6. Termination
You have the right to opt out of the Program at any time by contacting the Program Coordinator or writing to the Program at support@partizanhealth.com, or by contacting your Healthcare Professional.
We may also suspend or cancel your access to the Program at any time without advance notice if you breach these Terms or for the maintenance of the Program, although we will use reasonable efforts to provide you with notice wherever practicable.
7.Governing law
These Terms are governed in accordance with the laws of NSW, Australia.
8. Changes to these Terms
We reserve our right to modify these Terms at any time, including Program eligibility requirements. We will use reasonable endeavours to ensure that you are alerted to changes, such as contacting you via email. Your subsequent continued participation in of the Program after such changes have been made will constitute acceptance of the new Terms.
We may also update, improve or otherwise modify the Cohere Patient Support mobile application software and the Programs structure from time to time. If that happens, you may need to download or install updates of the software in order to continue using the Programs.

Cohere Patient Support Privacy Policy
We are committed to protecting the privacy of your personal information and to handling your personal information in a responsible manner in strict accordance with the Privacy Act 1988 (Privacy Act) and Australian Privacy Principles (APP).
This Privacy Policy describes how we collect, use and disclose your personal information.
1. Who is responsible for Personal Data processing?
The entity responsible for processing your Personal Data through the Cohere Patient Support Mobile Application is Partizan Worldwide Pty Ltd (Partizan Worldwide). Partizan Worldwide specialises in delivering tailored patient support programs and technology services designed to improve health outcomes for patients.
2. The Privacy Act 1988 and Australian Privacy Principles
The Privacy Act 1988 outlines the Australian l Privacy Principles, which set out standards, rights and obligations in relation to handling, holding, accessing and correcting personal information.
Partizan Worldwide is fully committed to complying with the Privacy Act 1988 and the Australian Privacy Principles.
3. Why do we collect information about you?
We collect your personal and health related information in order to administer the programs and services that you have consented to. When we collect sensitive information about you, we do so in strict accordance with the Australian Privacy Principles.
Your information may be collected over the phone, via email, via online portals or via the Cohere Patient Support mobile application.
The personal information collected and stored by us may include your name, date of birth, address, mobile phone number and email address. We may also collect personal information about your health, related medical condition and treatment requirements.
Occasionally, you may also provide information about other individuals to us such as your health care team including your doctor and allied health professionals. If so, we rely on you to inform those individuals that you are providing their contact details to us and to advise them that we can be contacted for further information. Where possible, we will give you the option of interacting with us anonymously.
We may hold details of program services that you use; when you request specific products or services, the status of the service delivery and the outcomes captured from that service interaction.
4. What information does the Application collect and how does it collect it?
All data is captured through the Cohere Patient Support mobile application and stored within our secure Health Management system via secure APIs. All data is strongly encrypted on our servers and accessed only by authorised individuals for the purpose of delivering the services. Refer to item 6 Security of your personal information below for more details.

4.2 Automatically collected information
The Application may collect certain information automatically, including, but not limited to, anonymous information about the way you use the Application. We use this information to help us continuously understand and optimise the experience for all users. The Cohere Patient Support mobile application uses analytics to measure anonymised activities and associate multiple sessions across multiple devices. We only use this data for the purpose of internal analytics to improve our Services. We do not share this information with any third parties.
5. When we disclose your personal information
In order to deliver the services you request, we may disclose your personal information to organisations outside of Partizan Worldwide Pty Ltd who are directly involved in the delivery of these program services. Your personal information is disclosed to these organisations only in relation to providing services to you.

Personal information that we hold in our systems is strongly encrypted and protected through the use of complex, secure passwords and multi-factor authentication. Access to personal information is restricted to those who are specifically granted authorised access to c administer program services that you have consented to.
Our staff are responsible for handling personal information in accordance with the Privacy Act 1988 and Australian Privacy Principles. We train our staff on the requirements of the Privacy Act 1988 and the Australian Principles so they are fully conversant with upholding the law and protecting your privacy.
We will keep your information so that we can continue to provide the products and services you have requested from us. We will destroy or permanently de-identify personal information that we no longer need to provide those services.
7. You can access your personal information
You have the right to access your personal information that we store as prescribed by law at any time. If you would like to do so, please contact us in writing by sending an email to privacy@partizanhealth.com.
8. Deletion of the Cohere Patient Support App
You may delete the Cohere Patient Support Application from your mobile device at any time. There are no costs or penalties associated with the deletion of the Application.
Deleting the Cohere Patient Support Application will permanently remove all data stored by the Application on your mobile device. All data stored in our healthcare management system will be retained for 10 years after the last date of modification, as required by law. Please see item 13 below for more detail.
9. If you have a complaint
If you have a complaint about the privacy of your personal information, please contact us in writing at privacy@partizanhealth.com. We will promptly acknowledge and investigate all complaints.

11. Rights of access and correction
You can access and update your information within the Application by logging into your account. You have the right to access and to request correction of any of your information provided to us in connection with your use of the services. If you would like to view the information relating to you that we have stored, or to request correction of such personal information, or if you have any concerns regarding your privacy please contact us at: privacy@partizanhealth.com.
12. What are my opt-out rights?
You can stop all collection of information by the Application easily by deleting your account in ‘Settings’. You can also request to have information corrected via email - privacy@partizanhealth.com.
To opt-out of all analytics we use, you can uninstall the Application or stop using our Services.
13. Data retention policy, managing your information
We will retain your data for as long as you use the Application. We retain users’ information for 10 years after last date of modification, as required by law.
We will retain automatically collected information for up to 24 months and thereafter may store it in aggregate where all data is anonymous.
If you’d like us to delete the data you have provided us, please contact us at privacy@partizanhealth.com and we will respond in a reasonable time. Please note that some or all of the user provided data may be required for the Application to function properly.
14. Changes
This Privacy Policy may be updated from time to time. We will notify you of any changes to our Privacy Policy by posting the new Privacy Policy here and informing you via email. You are advised to consult this Privacy Policy regularly for any changes, as continued use is deemed approval of all changes. You can check the history of this Policy by the revision numbers at the end of the Policy.

By agreeing to this Privacy Policy, you are consenting to the collection and processing of your information as set forth in this Privacy Policy now and as amended by us.
16. Limitation of this Privacy Policy
This Privacy Policy does not extend to third party websites beyond our control, that you may access via links in the mobile Application.
